Abstract
Health care for people with comorbidities that require treatment from multiple specialists is a challenge in health systems. In this paper, we analyze the process of continuity and discontinuity of care, that a woman received for a neck tumor at the three levels of atention in the Instituto Mexicano del Seguro Social. We performed a documentary and interpretive analysis of the Electronic Medical Record (emr) and the patient’s narratives. The case revealed a fragmented care system, where doctor-patient communication is brief, a multiplicity of versions of the medical record; limited access to the emr depending on the position, staff´s lack of knowledge, overlap with the physical record and poor communication between levels and care services
